Technology is enabling patient engagement and driving superior outcomes like never before. While the benefits are clear, it can be challenging and cost-prohibitive to develop patient-centered technology. A key advantage of strategic partnership is gaining access to innovative tools without the heavy lift and burden of creating them from the ground up.
We provide access to the industry’s leading technology solutions, including programs that are proprietary to Lifepoint and not offered by competitors.
RehabTracker is a groundbreaking person-centered mobile app transforming how our partners engage with patients and their families. Combined with our approach of focusing our care around What Matters to You (the patient), RehabTracker allows patients and their invited loved ones to view their personalized goals, see and track their patient progress in real-time as logged by our therapists, including overall progress toward discharge goals, activities of daily living, distance and transfers, share that progress with invited family and friends, share patient photos during their recovery journey, receive comments from staff and messages of support from loved ones – all from the palm of their hands. Areas of patient progress include self-care activities such as bathing, dressing, and eating, mobility activities including walking, transfers, and bed mobility, speech-language activities including expression and comprehension, memory and problem-solving, and diet/swallowing food and liquid consistencies. Facilities utilizing RehabTracker have experienced greater levels of patient motivation as well as patient and family member engagement and satisfaction.
RehabTracker PRO goes hand-in-hand with the RehabTracker app to drive patient and family engagement in the individual’s care and recovery. With RehabTracker PRO, therapists can log and illustrate the progress each patient makes toward their personalized goals, and track and show that progress in real time. The app measures activities of daily living, distance and transfers in a patient-centric scoring method.
Ekso NR™ suit is the first robotic exoskeleton designed for neuro-rehabilitation patients recovering from neurological deficits like stroke and spinal cord injury. The NR™ suit is a wearable, bionic exoskeleton that augments strength to help patients stand and relearn to walk independently, improving their step patterns, weight shifting and posture. It also helps to mobilize patients earlier in their rehabilitation. Research shows that the NR suit helps improve walking distance, balance and overall patient satisfaction.
Rehab Relay is Lifepoint Rehabilitation’s proprietary pre-admission screening software that simplifies the admissions process for case managers, aids with compliance and ultimately helps patients access the right care to reach their recovery goals at the right time and the right place.
Relay features a streamlined data entry workflow, visual indicators that show screening progress and security integration that permits users to submit security requests to drive application and facility access. Relay’s intuitive interface allows for ease of use and correct documentation, and its push notification feature provides gentle reminders about the patient admission process and compliance deadlines.
Rehab Relay is inclusive of MedPar data which tells the program what their primary and secondary market consists of and enables drill-down by rehabilitation patient profile.
My Rehab Viewer is a compact website that serves as an online brochure that representatives from the inpatient rehabilitation locations can show to prospective patients and referral sources. Viewers can quickly learn about the conditions treated in inpatient rehabilitation, what to expect and bring, and what that particular unit or facility offers in terms of services, amenities, support groups and more.
Rehab Therx is a Lifepoint Rehabilitation exercise app for use on the iPad Mini. The app allows therapists to interact with patients as they define their exercise program, tailoring them to each patient’s needs. The app has over 300 exercises and includes yoga and built-in patient education. Exercises can be printed in either English or Spanish and it also provides the ability to e-mail the exercise program to the caregiver, family or patient, keeping all parties actively engaged in the care experience.
Lifepoint Health’s proprietary Electronic Health Record (EHR) system, Pinpoint, efficiently compiles and stores vital patient data. Integrated with an interdisciplinary team approach, Pinpoint provides a fully cohesive system that houses all documentation for a behavioral health facility.
This system can be incorporated with other EHR systems, creating a seamless flow from inpatient care, intensive outpatient treatment (IOP), partial hospital programs (PHP) and more, to a patient’s next level of appropriate care.
Pinpoint also facilitates outcome measure tracking so changes in a patient’s depression as well as symptoms associated with trauma and anxiety, can be measured from admission to discharge. This detailed approach, coupled with electronic health information exchanges (HIEs), provides insight into relevant and actionable patient data trends.
